Good Faith Estimate

As part of our commitment to transparent and ethical healthcare practices, we provide a Good Faith Estimate for the anticipated costs of therapy services to all our clients who choose private pay or out-of-network services. This estimate is designed to offer a clear understanding of the expected expenses related to therapy sessions. Please keep in mind that this is a general estimate, and actual costs may vary depending on individual needs.

Estimated Costs:

  • Session Fee: Our standard session fee for private pay clients is $185 per individual session.

  • Session Frequency: Therapy sessions are typically scheduled on a weekly or bi-weekly basis.

Creative Source Counseling recognizes every individual’s mental health treatment journey is unique and personalized. How long you need to engage in mental health services and how often you attend sessions is influenced by many factors, including, but not limited to:

      • Your schedule and life circumstances

      • Your provider’s availability

      • Ongoing life challenges

      • The nature of your specific challenges and how you address them

      • Personal finances

 

Insurance Information (if applicable):

For clients seeking out-of-network reimbursement, we will provide you with a detailed invoice and necessary documentation (Known as a Superbill) to submit to your insurance provider for potential reimbursement. Please verify your insurance benefits and out-of-network coverage with your insurance provider.

Total Estimated Client Responsibility:

The estimated client responsibility will depend on your specific needs and whether you plan to seek out-of-network reimbursement from your insurance provider. We will work with you to determine your exact costs.

This estimate is based on the information available at the time of scheduling and may change based on the actual services provided. We encourage you to contact our office for a personalized estimate tailored to your circumstances.

The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or special circumstances occur and you will be provided with a new Good Faith Estimate should this occur. If this happens, federal law allows you to dispute (appeal) the bill if you and your provider have not previously talked about the change and you have not been given an updated Good Faith Estimate.
